Please read all instructions carefully as the process has changed significantly from prior years.
Renewal of a USA Swimming Club REQUIRES that an authorized club representative sign and date the application for Club renewal and send either the SIGNED HARD COPY to AZSI via regular postal mail, OR if signed electronically via email to firstname.lastname@example.org.
By USA Swimming rules, if you do not sign the application and send all six pages to AzSI office your club membership cannot renew. No exceptions or grace periods.
This form can either be printed out, completed and returned to AZSI via regular postal mail OR can be filled out and signed electronically and returned to AZSI as a PDF attachment in an email.
ONLINE CLUB RENEWALS ARE NO LONGER AVAILABLE
Payment for club renewal can be submitted by either check to Arizona Swimming, or through the online payment portal below.
